Úlcera marginal após bypass gástrico com Y de Roux em cirurgia bariátrica : a propósito de dois casos clínicos

Resumo:Obesity is a disease that affects approximately 710 million people worldwide (1). Surgical treatment for obesity is indicated in patients with Body Mass Index>35kg/m2 and comorbidities associated with obesity, such as Diabetes Mellitus type 2 and Arterial Hypertension, or in patients with Body Mass Index>40kg/m2 without comorbidities. Amongst the available surgical techniques used in Bariatric Surgery, Roux en Y Gastric Bypass was the second most used technique in 2019 (35,3%, meaning 295000 surgical procedures worldwide) (2). This fact makes it mandatory for a surgeon to know its possible complications. One of those complications is Marginal Ulcer. A Marginal Ulcer refers to the development of mucosal ulceration at the gastrojejunal anastomosis, typically on the jejunal side. Marginal Ulcer after Roux en Y Gastric Bypass is the theme of this thesis, whose purpose is to elaborate a revision of the available literature and a reflection about the clinical importance of Marginal Ulcer in the preoperative evaluation and postoperative management of patients submitted to Roux en Y Gastric Bypass. Topics such as erradication of H. Pylori prior to surgery, duration of the use of Proton Pump Inhibitors in the postoperative or when should Upper Digestive Endoscopy be done in the postoperative, will be developed in this thesis. The thesis is going to start with an introduction followed by the discussion of two clinical cases and will end with a literature review about Marginal Ulcer.
