The methodology used fits into the qualitative paradigm. A semi-structured interview was the instrument used to collect data and information. 14 special education teachers with special education students that are part of an individual Transition Plan were interviewed. The interview was done individually and occurred in the natural school environment, in 10 groupings in the Amadora district. We also used a questionnaire to demographically characterize the participants in the study. In analyzing the data, we were able to learn that schools that implement the Individual Transition Plan and try to provide different answers, in accordance with the specific needs of each special education student; however, they have difficulty in finalizing the several phases of the transition process, especially when it comes to placing the students in professional internships or a work environment
