    Pancreatites agudas graves: Estudo retrospetivo da casuística do serviço de Medicina Intensiva da ULS São João 2018-2022

    Introduction: Severe acute pancreatitis (SAP) - pancreatitis associated with organ failure and/or local complications that lasts longer than 48h - accounts for 20-30% of acute pancreatitis and its mortality rate is approximately 15%. An early and adequate stratification of severity, with definition of those who benefit from admission in Intensive Care Medicine Services (ICMS) is decisive for the outcome. This study aims to understand the epidemiology of SAP and establish the factors associated with poor outcomes. Methods: Retrospective longitudinal observational study of patients admitted due to SAP to the ICMS of a tertiary hospital, between January 1st, 2018 and December 31st, 2022. Multiple variables related to patient, disease and treatment were studied and the chosen outcome variables were: "Death", "Number of hospitalization days" and "Number of days in the ICMS". Results: We studied 121 patients diagnosed with SAP, 1,05% of total hospitalizations in the ICMS. The median SOFA was 4, SAPS II was 32 and 96,7% of patients had a moderately severe or severe form according to the Atlanta classification, with invasive mechanical ventilation, vasopressors, renal support and parenteral nutrition used in 22, 20, 10 and 11% of patients, respectively. The median length of stay in the ICMS and in the hospital were 3 and 13 days, respectively. Hospital mortality rate was 19% (standardized mortality based on SAPS II of 1,46). The variables independently associated with length of hospital stay were: presence of pathogens in abdominal samples, use of antifungals, presence of pancreatic imaging abnormalities, use of parenteral nutrition and number of days of hospital stay pre-ICMS. Only SAPS II, pancreatic puncture/drainage and development of intra-abdominal hypertension were significantly and independently associated with hospital mortality. Conclusions: The morbi-mortality associated with SAP is high. Severity at admission in ICMS, delay in admission to ICMS, digestive dysfunction and development of infection or of intra-abdominal hypertension associated with SAP appear to be associated with a worse clinical outcome.     Self-Consolidating Concrete Produced With Fine CDW

    Self-consolidating concrete is a concrete technology that seeks to simultaneously achieve fluidity, passing ability and segregation. In its manufacture there is the possibility of using alternative materials, such as ground ceramic, predominantly in construction and demolition waste. Therefore, this study aims to analyze the feasibility of this concrete regarding its properties in the fresh and hardened state when there is replacement of fine sand by ground ceramic aggregate. For this, the flowing tests, flow time, visual stability index, J ring, L box, V funnel and the molding of specimens in three different compositions were carried out, one being the reference compositions and the other two with replacements of 25 and 50% in the natural fine sand by the ceramic waste sand. The mixes in which the replacements were carried out presented good fluidity, without of segregation, however, the passing ability of the mix with 50% of replacement did not fit within the recommended range.     Occupational Exposure of Firefighters in Non-fire Settings

    This work assessed firefighters exposure to particulate matter (PM) in non-fire work settings during pre-fire season, as a baseline for the respective occupational exposure characterization. Indoor and outdoor air sampling was conducted for two weeks in pre-fire season of 2021 in seven fire corporations (FC1-FC7) in north of Portugal. PM fractions (PM2.5, PM10) were continuously monitored concurrently in indoors (living rooms, rest areas, truck bays) and outdoors. The results showed low levels of pollution. Indoor PM10 was between 2 and 205 g m3 (mean 10 g m3); PM2.5 were 2115 g m3 (8.5 g m3). Both indoor PM fractions were highly and significantly correlated (rs = 0.9590.997). PM2.5 accounted for 85% of indoor PM; indoor to outdoor ratios (I/O) of PM2.5 ranged between 1.4 and 3.0, thus emphasizing the contribution of indoor emission sources for fine fraction. Outdoor PM10 were 26 times higher than indoors (6894 g m3; mean 21 g m3); and 29 times higher for PM2.5 (5 169 g m3; 5 g m3). Outdoor PM were moderately correlated (rs = 0.584 0.878), most likely due to meteorological conditions. Coarse particles contributed the majority of ambient PM10 and accounted for 79%.     Minimal time delivery of multiple robots

    Consider a set of autonomous vehicles, each one with a preassigned task to start at a given region. Due to energy constraints, and in order to minimize the overall task completion time, these vehicles are deployed from a faster carrier vehicle. This paper develops a dynamic programming (DP) based solution for the problem of finding the optimal deployment location and time for each vehicle, and for a given sequence of deployments, so that the global mission duration is minimal. The problem is specialized for ocean-going vehicles operating under time-varying currents. The solution approach involves solving a sequence of optimal stopping problems that are transformed into a set variational inequalities through the application of the dynamic programming principle (DPP). The optimal trajectory for the carrier and the optimal deployment location and time for each vehicle to be deployed are obtained in feedback-form from the numerical solution of the variational inequalities. The solution is computed with our open source parallel implementation of the fast sweeping method. The approach is illustrated with two numerical examples.

    Eduardo Souto Moura e Paulo Mendes da Rocha - O Projecto Moderno Hoje.

    This study tries to understand how the architects Eduardo Souto de Moura and Paulo Mendes da Rocha, perceived in this work as representatives of the Portuguese and Brazilian architecture, answer to the moment of criticism of the Modern Movement, in comparison with the international scene. To appreciate the relevance of the two paths it is important to highlight the features that define them, in order to understand the reason why this movement is considered as a valid option for nowadays. Therefore, a presentation of the themes that dominate and influence their design decisions, and consequently their projects, is followed by a retrospective of the Modern Movement in each country and by an attempt to compare them. It is fundamental to consider the antecessors of Souto de Moura and Mendes da Rocha, in order to achieve an improved perception of the modern reality in the two countries. These include Ruy Athouguia, Fernando Távora, Lucio Costa, Óscar Niemeyer and Vilanova Artigas. The choice of these architects is justified by the nature of their set of principles, which will influence the development of the modern architecture in these particular contexts. A reflection on the paths paved by Eduardo Souto de Moura and Paulo Mendes da Rocha is performed under these lines, where their individual experience, influences from previous (Portuguese and Brazilian) architects, and contributions from the masters of the Modern Movement, namely Le Corbusier and Mies van der Rohe, are implicit. It will be shown in this dissertation that these directions can be useful as a model for other architects that aim to reestablish an architectonic thinking suitable to the problems that affect contemporary society.

    In the last few years, European countries and its citizens have suffered from the severe impact of the financial and economic crisis, caused by a combination of international and national factors. In Portugal, the crisis has led to a number of consequences, such as the rise in unemployment, wage and social benefit cuts, tax hikes, and the impoverishment of different population groups, particularly the most vulnerable, largely concentrated in the urban areas. The research presented in this paper focuses firstly on Portugal at a national level, in 2007 and 2014, followed by an analysis at the metropolitan level, based on indicator figures, intended to explore social vulnerability in two territorial units (Functional Region of Porto and Functional Region of Lisbon). The dynamics in times of crisis (2007 and 2014) are analysed, illustrated by the two profiles of social vulnerability identified.
